What Causes Dermal Filler Migration?

Dermal filler migration happens when material moves away from the area where it was originally placed. It’s considered an uncommon complication, and factors such as injection technique, filler volume, pressure, facial movement, and the filler’s characteristics have been proposed as contributors.

Freya’s Lair Medical Spa offers cosmetic injectables, including dermal fillers, in Albuquerque, NM. The practice features Olga Lucero, MSN, FNP-C, a certified family nurse practitioner and experienced aesthetics provider, along with a team of nurse practitioners and estheticians.

Why Can Dermal Filler Move?

True migration means filler has moved beyond its intended anatomical area. It shouldn’t be confused with temporary swelling, unevenness, or other changes that sometimes occur shortly after injections.

Several factors have been associated with migration:

  • Placement depth: Filler placed in an inappropriate tissue layer can be more prone to unwanted movement or visible irregularities.
  • Too much volume: Overfilling an area may put additional pressure on the surrounding tissue.
  • Injection pressure: Injecting a large amount quickly or under excessive pressure can contribute to displacement.
  • Facial movement: Highly mobile areas, such as the lips, experience repeated muscle movement that may influence how filler settles over time.
  • Pressure after injections: Aggressive massage or repeated pressure on the area can contribute to unwanted displacement in some circumstances.
  • Product characteristics: Different fillers have different firmness, flexibility, and flow characteristics, so the product needs to match the treatment location.

Migration has been reported with several types of facial fillers, including temporary hyaluronic acid fillers, although it remains an uncommon complication

What Does Filler Migration Look Like?

Signs can depend on the location and amount of displaced filler. You could notice fullness outside the original injection area, asymmetry, a lump or ridge, or a contour that looks different from what you expected.

Not every bump after filler means the product has migrated. Early swelling and temporary irregularities may occur while the area settles. Persistent or new changes that appear later deserve an evaluation rather than repeated pressing or massage at home.

Can Migrated Filler Be Corrected?

Many migrated hyaluronic acid fillers can be reduced or dissolved with hyaluronidase, an enzyme that breaks down hyaluronic acid. Other types, including calcium hydroxylapatite, poly-L-lactic acid, and polymethylmethacrylate, don’t respond to hyaluronidase and require different management strategies.

Some temporary or biodegradable fillers gradually break down over time. Permanent fillers such as PMMA don’t naturally dissolve, and persistent migration sometimes requires additional intervention or, in select cases, surgical removal.

The right next step depends on the type of filler, where it has moved, how long it has been present, and what’s actually causing the change. An in-person assessment is important before attempting correction.
